Membership Plans
Family Co-Membership
The Primary Member may designate immediate family members as authorized users to enjoy covered membership privileges.
Benefits remain under the Primary Member’s account and are:
• Non-transferable
• Non-cash convertible
• Not shareable as accumulated points or balances
Upon the passing of the Primary Member, an authorized immediate family member may become the new Primary Member, subject to the program’s prevailing rules and regulations.

1. Membership is available only upon approval by Big Hearts Adult Daycare & Assisted Living, Inc.
2. Membership fees are non-refundable except where required by law.
3. Care Points have no cash value and are non-transferable.
4. Care Points may only be redeemed for qualified Big Hearts services, subject to assessment, availability, and program rules.
5. Membership benefits may only be used by the Primary Member and authorized immediate family users under the Family Co-Membership Program.
6. Benefits cannot be exchanged for cash.
7. Discounts cannot be combined with other promotions unless expressly stated.
8. Emergency Care Benefits are provided through accredited partners and remain subject to their eligibility requirements, coverage limits, exclusions, and terms.
9. Dental Care benefits are provided through accredited partner clinics and are subject to availability.
10. Healthcare benefits may require prior appointment scheduling.
11. Big Hearts reserves the right to substitute partner providers without prior notice.
12. Certain services may require medical assessment before acceptance.
13. Membership does not guarantee admission to any assisted living facility.
14. Admission remains subject to medical assessment, availability of rooms, and facility policies.
15. Big Hearts reserves the right to amend the Membership Program, benefits, participating partners, and these Terms and Conditions without prior notice, provided such changes comply with applicable laws and regulations.
16. Membership benefits are subject to applicable taxes, laws, government regulations, and partner policies.
17. Abuse, fraud, or misuse of membership privileges may result in suspension or termination of membership.
18. The Big Hearts Membership Program is not an insurance policy, HMO, investment, savings account, or pre-need plan.
